Why Do Wisdom Teeth Need to Be Removed?

Wisdom teeth are removed when there is not enough room in the jaw for them to come in straight. Crowded or impacted third molars can damage neighboring teeth, trap bacteria, cause infection, and shift the alignment of teeth that were previously straight.

The jaw simply stopped getting bigger. Third molars arrive last, usually between 17 and 25, into a mouth that is already full. When there is no room, the tooth either stays trapped in the bone, comes in at an angle, or emerges partway.

A partially emerged wisdom tooth is the worst of the three. The flap of gum over it traps food and bacteria in a space no toothbrush reaches, which causes repeated infection called pericoronitis.

An impacted wisdom tooth is one that fails to emerge, or emerges only partially. Impacted teeth angled forward push against the second molar, which can damage its root or cause decay where the two teeth meet below the gum line.

Not every wisdom tooth needs to go. If yours came in straight, have room, and you can clean them, we leave them alone and watch them.

What Is the Difference Between Wisdom Teeth Removal and a Regular Tooth Extraction?

A regular tooth extraction removes a visible, fully erupted tooth and usually takes minutes. Wisdom teeth removal often involves teeth that are impacted or angled, which can require opening the gum and sectioning the tooth, and takes longer to heal.

Regular extraction is the answer for a tooth that cannot be saved: a tooth broken below the gum line, one lost to advanced decay, or one that has failed after a root canal. The socket heals over a few weeks and the tooth gets replaced with an implant, bridge, or partial denture.

Wisdom teeth are the exception. They do not need replacing, because there was not room for them in the first place.

The extraction itself does not hurt, because the area is fully numb. Most patients report soreness and swelling for 3 to 4 days afterward, peaking around day 2. Laughing gas and oral conscious sedation are available for the appointment itself.

What Does Recovery From Wisdom Teeth Removal Look Like?

Most patients recover from wisdom teeth removal in 3 to 5 days, with swelling peaking around day 2. Plan to take it easy for 48 hours, eat soft foods for about a week, and avoid straws and smoking for at least 72 hours.

The first 24 hours. Bite on gauze until bleeding stops, use ice on and off, and rest. Do not rinse forcefully, spit hard, use a straw, or smoke. All four can dislodge the blood clot in the socket.

Days 2 and 3. Swelling peaks and then starts to go down. Warm salt water rinses begin. Soft foods only.

Days 4 through 7. Most patients are back to normal activity. Stitches, if used, dissolve on their own.

Dry socket is the complication to know about. It happens when the clot protecting the bone is lost, usually from suction or smoking, and it produces a deep ache two to four days after surgery. It is treatable, and it is mostly preventable by following the first-24-hours rules.
